Adolescence can be a turbulent time for any young person but even more daunting if you have the added stress of having to cope with a chronic illness or disability. The GMCT Transition Care Program is a new initiative aimed at improving continuity of care for young people with chronic health problems as they move from paediatric to adult health services. Originally started off as a Yahoo! Email Group a couple of years ago, our Young Adults online support network has migrated to Facebook. This is a moderated group so that everyone can feel safe and protected in this online environment. Young adults who are under the age of 35 are all encouraged to join this group to connect with others who know exactly what you are going through.
